Goi Referral Process
At the beginning of the fall and spring semesters, we will be accepting enrollment for the GOI mentorship program. The steps for referring a student are listed below:
The GOI Coordinator will communicate with you should there appear to be any delay in pairing. You're welcome to email me to check in about the process at any time.
- Complete the Grief Outreach Initiative Referral Form to convey background information about the student and situation (e.g., grade of mentee, circumstance of loss) to ensure their appropriateness for the program and help with the mentor-matching process.
- Share the parental consent form with the students' parents and have them return the signed form. (Download document below.)
- Email photo or a scanned copy of the completed parental consent form (signed by students’ parents/guardians) to [email protected]. We cannot make placements until we have this form signed and in hand.
- The GOI Coordinator, Beth Anderson Walker, will follow-up with you via phone or email once we receive the parental consent form regarding any further information about the student to make the most appropriate pairing.
- Simultaneously, the referred child will begin paired with a mentor. This will be done as quickly as possible, but the timing may vary depending on the time of the referral and number of available mentors.
